When should you remove wisdom teeth in Shrewsbury, PA?

Whenever wisdom teeth have no room to erupt and enter the mouth as fully functional teeth we will recommend removal. We advise waiting long enough to determine if the teeth will come in normally prior to making the decision to remove wisdom teeth. Wisdom teeth will erupt between the ages of 16 and 19 usually. We recommend a panoramic x-ray image be made to assess the development of wisdom teeth to determine if removal is the right decision for you.

Wisdom teeth are the 3rd molars in every quadrant of your mouth and often there is just no room for them to come in completely or they can become impacted wisdom teeth if they hide under the gum (soft tissue impaction) or under the bone (full bony impaction). The risk of impacted wisdom teeth is that bacteria can find a way under the gum and teeth causing decay and infection of the area and often risking the health of the surrounding teeth as well.

When we see that wisdom teeth are coming into alignment properly we advise that the best choice is to clean them effectively and keep them as useful chewing teeth just like any other. We are conservative in our approach and do not recommend extraction for all of our patients. In this new era of dental implant tooth replacements, dentists have to consider what the patient wants to do after an extraction is performed. Often a dental implant or a bridge will be considerations for filling the gap where a tooth was lost. The results of a tooth extraction is alveolar bone loss- the bone shrinks away! Research has shown that up to 50% of the supporting bone necessary for anchoring an implant will dissolve and atrophy in the first year alone. If a patient is planning on an implant at any time during the future, alveolar ridge preservation or ridge preservation grafting should be planned along with your extraction. How is dental bone grafting done? Between .5-1.0 cc of bone graft material is placed in the extraction site and is usually covered by a surgical semi-permeable membrane that was originally Gore-tex and is now either a collagen or a Teflon perforated membrane that permits bone growth to occur while eliminating the tendency for the body to allow gum tissue to fill the socket and result in lost bone. Understanding how bone healing works allows us to perform guided bone regeneration that preserves and can augment sites where we plan to place dental implants. The area is closed with sutures and allowed to rest for 3 weeks on average. A second appointment will be made for suture removal and you may have your implant placed as early as 6 to 8 weeks after your extractionsocket grafting procedure.

When patients have decided that keeping their original teeth is no longer possible, a denture or implant denture is a common choice these days. We prefer to coordinate the entire process of creating an immediate denture and removing the teeth at the same time for our patients. The process is different from just removing one tooth.

Multiple teeth being extracted at one time will require alveoloplasty (smoothing the underlying bone) or the gum tissue will not be smooth when the gums heal and the denture sore spot will become a huge problem if you could wear a denture at all. Dental bone grafting is often considered when extracting all the teeth to help preserve and rebuild the bone that will support a denture or an implant retained denture. The gums are sutured in the proper position for the next 2 -3 weeks and finally your immediate teeth placed.

Our patients have new “teeth in a day” but must wait for full healing prior to us moving forward to final restorations. Pain medications and antibiotics will often be prescribed and patients can be back to normal life in a few days. Final healing of the gums may take several months. We oversee the healing for the entire post-operative period. Transitioning out of your original teeth and into new teeth is a journey and we are here for you along the way to help you achieve the new smile you want and deserve.

Tooth extraction is considered oral surgery when an incision is made in the gum to remove a tooth. The incision site will then need to be closed with stitches. An impacted tooth can create problems later. An oral surgeon may be called in for the procedure.

It is recommended to allow yourself 48 to 72 hours rest after surgery to let your mouth heal properly. After that, any pain and swelling should be minimal. Work and physical activity can be resumed.
A tooth infection that spreads into the body presents as redness, swelling, and pain. Fever, fatigue, flushed skin, and headaches may also accompany a spreading infection. Spreading infections require immediate attention and are life threatening in every case. Infections can quickly spread to the brain or close off your airway resulting in hospitalization or death. If you have a large swelling or a spreading infection you will need to see us immediately. If you cannot see us then consider getting to the emergency room for the care you need.
Recovery from a surgical tooth extraction takes anywhere from 2 to 4 days for the gum to clot and swelling to subside. After that, the patient can go about business as usual. The extraction site should be fully healed 2 to 3 weeks after surgery.
